DECLARATION

I understand that by accepting any funding granted, I am agreeing to use this only for the purposes outlined in this application. Any funds not spent will be returned to Music Therapy New Zealand. I also accept as a condition of the funding, that Music Therapy New Zealand may request information relating to the use of the grant, and I agree to provide this if asked. This may include, but is not limited to, financial records, invoices, and receipts.

Music Therapy has not only brought our daughter great improvements in motor planning, finger strength, communication and attention span, it has brought her great joy as well. There is no place she would rather be than in a room making music.
